Responsibilities
- Coordinate accurate and timely resident assessments in a skilled nursing and rehab setting.
- Complete MDS/RAI assessments and develop individualized care plans.
- Collaborate with the interdisciplinary team to ensure regulatory compliance and positive clinical outcomes.
- Utilize EHR systems to support documentation and survey readiness.
- Contribute to quality improvement initiatives.
- Support ongoing education and professional growth.
Qualifications & Skills
- Licensed Practical Nurse (LPN) license.
- Knowledge of MDS/RAI assessments and regulatory standards.
- Strong collaboration and communication skills.
- Ability to work effectively in a fast-paced, 24/7 environment.
- Proficiency with Electronic Health Records (EHR) systems.
