New London Nursing and Rehab is a skilled nursing and rehabilitation center committed to high-quality, resident-centered care and strong clinical outcomes. We are currently seeking an experienced and detail-oriented MDS Coordinator to join our team.
About the role
This role is ideal for a clinician looking for a balanced schedule while continuing to play a key role in care coordination and compliance. This is a facility-based, in-house position only.
Responsibilities
- Coordinate and complete accurate, timely MDS assessments in accordance with state and federal regulations
- Manage the RAI process and support interdisciplinary care planning
- Ensure documentation supports quality outcomes and reimbursement
- Collaborate with nursing, therapy, and administrative teams
- Assist with audits, surveys, and quality improvement initiatives
Requirements
- Active RN or LPN license in good standing (required)
- Prior MDS experience in a long-term care setting (preferred)
- Strong knowledge of Medicare/Medicaid guidelines and RAI requirements
- Excellent organizational skills and attention to detail
- Ability to work independently and manage deadlines
