MDS Hybrid Position

About the Role

We are seeking a highly skilled and experienced MDS Hybrid Coordinator to join our team at Fletcher Rehabilitation and Healthcare Center. As a Hybrid MDS Coordinator, you will play a crucial role in the coordination and management of our nursing care plans, ensuring that our residents receive the highest level of care possible. This is a full-time position, with a blend of onsite and remote work (3 days onsite, 2 days remote).

Responsibilities

• Keep abreast of current federal and state regulations, as well as professional standards.
• Assist with the development of comprehensive care plans for residents in coordination with the MDS that accurately addresses the needs of the resident.
• Coordinate, manage, and monitor the written plan of care for each resident of the facility that identifies the needs of the resident and goals to be accomplished for each need.
• Monitor resident status changes to ensure appropriate and timely nursing or clinical team involvement.
• Assure MDS and support documentation are accurate representations of the resident and meet regulatory and auditor requirements.
• Perform regular audits of documentation to assure accuracy.
• Assist nursing management with the discharge process.
• Perform administrative requirements, such as completing necessary forms and reports.
• Assure that established infection control and standard precaution practices are maintained when providing care.
• Follow established safety precautions when performing tasks and using equipment and supplies.
• Maintain strict confidentiality regarding sensitive health information of residents.
• Report all hazardous conditions, damaged equipment, and supply issues to appropriate persons.

Requirements

• Current state license as a nurse.
• CPR certified.

Qualifications

• One year of experience as a nurse in a long-term care setting (preferred).
• Knowledge and training in all aspects of the MDS process.
