About the role
Warsaw Rehabilitation and Healthcare Center, located in Warsaw, NC, is a Long Term Care facility that provides quality care to its residents. They are currently hiring a Full-Time MDS Coordinator, with 3 days on-site and 2 days remote.
Responsibilities
- Keep abreast of current federal and state regulations, as well as professional standards.
- Aid in the development of comprehensive care plans for Residents in coordination with the MDS that accurately address their needs.
- Coordinate, manage, and monitor the written plan of care for each Resident of the facility.
- Ensure that the care plan identifies the needs of the Resident and sets goals for each need.
- Collaborate with nursing management to coordinate, manage, and review nurse's notes to ensure the care plan is being followed.
- Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
- Audit MDS and support documentation to ensure accuracy and compliance with regulatory and auditor requirements.
- Assist in the discharge process, including completing necessary forms and reports.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Report hazardous conditions, damaged equipment, and supply issues to appropriate personnel.
- Maintain the comfort, privacy, and dignity of Residents and interact with them in a warm, respectful, and caring manner.
- Respond promptly and courteously to call lights when working in Resident care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
Requirements
- Current State License as a Nurse
- CPR Certification
- Prior MDS Nurse experience (preferred)
- One year experience as a Nurse in a long-term care setting (preferred)
