MDS Director

Position: MDS Director (RN) Facility: Beach Breeze Rehab and Care Location: West Palm Beach, FL Status: Full-Time Compensation: $45.00–$49.00 per hour (based on experience) Position Summary Beach Breeze Rehab and Care is seeking an experienced and highly organized MDS Director (RN) to lead the facility’s MDS program and ensure accuracy, compliance, and excellence in resident-centered care. This leadership role is essential in supporting appropriate clinical documentation, reimbursement optimization, and interdisciplinary collaboration. Key Responsibilities Oversee accuracy, completeness, and timeliness of all MDS assessments and supporting documentationEnsure compliance with state and federal regulations, including Medicare and PDPM requirementsConduct comprehensive resident assessments and interviewsDevelop, implement, and monitor individualized care plans for short- and long-term residentsLead and participate in interdisciplinary care plan meetingsCollaborate with nursing staff, physicians, therapists, specialists, residents, and families to improve outcomesProvide leadership, guidance, and ongoing education to nursing staffConduct in-service training on MDS, RAPS, triggers, PDPM, and Medicare guidelinesMaintain and manage MDS and care plan scheduling for all residents Qualifications License: Active, unencumbered Florida RN licenseExperience: Minimum 2 years of MDS Coordinator experience in a Skilled Nursing FacilityStrong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning processesExcellent leadership, organizational, and communication skillsAbility to manage deadlines and maintain regulatory compliance in a fast-paced environment Benefits Comprehensive Medical & Dental CoverageGenerous Paid Time Off (PTO)401(k) Retirement PlanEmployee Recognition ProgramSupportive, team-oriented work environmentCompany perks and employee discounts Why Join Beach Breeze Rehab and Care?
