We are looking for an Interim/PRN MDS Coordinator to join our fun, team-oriented facility in Raymore, MO (just east of Belton near HWY 58 & Sunrise Dr). At Sunrise Nursing & Rehabilitation, we provide specialized care and therapy to help residents regain strength, improve mobility, and transition back to independent living. Our physician-supervised, round-the-clock care meets the unique needs of each resident, including those with complex chronic conditions.
Responsibilities
- Complete the RAI process and care management from admission to discharge in accordance with company policy, federal, state, and certification guidelines (e.g., Minimum Data Set, discharge/admission tracking).
- Coordinate information systems operations and education for the clinical department under the direction of the Director of Nursing and VP of Clinical Reimbursement.
- Collaborate with the Interdisciplinary Team to assess resident needs and provide schedules for MDS assessments and care plan reviews.
- Ensure the Interdisciplinary Team makes decisions on additional MDS assessments based on clinical criteria from the RAI User’s Manual.
- Assist with daily stand-up meetings, including reviewing resident care and setting assessment reference dates.
- Comply with federal and state regulations for the RAI process and monitor MDS/care plan documentation for accuracy.
- Maintain current MDS status of assigned residents per state and federal guidelines.
- Accurately enter resident information into computerized MDS programs and code MDS using medical records, observations, and interviews with staff, residents, and family members.
- Attend interdisciplinary team meetings, quality assurance meetings, and other gatherings to gather information and update records.
- Assist the Director of Nursing (DON) with identifying significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect updates.
- Prepare scheduling and notices for resident care planning conferences and communicate outcomes to staff, residents, and responsible parties.
- Stay updated on data entry and computer technology related to MDS processes.
- Submit required documentation electronically to state databases and ensure timely MDS submissions with follow-up on validation errors.
- Maintain validation records systematically and participate in regulatory or certification surveys.
- Report audit trends to the Quality Assurance Committee and ensure completion/timeliness of the RAI process.
- Correct and submit final MDS assessments to appropriate state and federal agencies.
- Assign, assist, and instruct staff in the RAI process, PPS Medicare, Medicaid (Case Mix), and clinical computer systems.
- Maintain confidentiality of all necessary information.
- Perform other duties as assigned or required.
Requirements
- Graduate of an approved Registered Nurse program and licensed in the state of practice (required).
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
- Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
- Thorough understanding of the Quality Indicator process and OBRA regulations.
- Knowledge of the care planning process and MDS 3.0 (preferred).
