Responsibilities
- Completes minimum data set assessments and creates comprehensive plans of care after reviewing the patient’s medical record and communicating with direct care staff, the patient’s physician, and family.
- Develops and completes the patient assessment process in accordance with federal and state regulations and company policies and procedures.
- Makes independent decisions on a regular basis regarding patient assessments.
- Works effectively with an interdisciplinary team.
- Uses basic computer skills to support patient assessments.
- Communicates clearly both orally and in writing with patients, families, and other healthcare professionals.
Requirements/Qualifications
- Virginia RN license.
- Skilled nursing and rehabilitation environment experience.
- Strong clinical skills.
- An understanding of the MDS 3.0 process.
- A commitment to service excellence.
