$87,800-$99,700/salary RN MDS Coordinator, full-time, at Lutheran Home in Arlington Heights, IL. Pay is based on experience. You'll make sure every resident's assessment accurately reflects their real physical, mental, and psychosocial status, work that directly determines the care plan they receive and how the facility is reimbursed for that care. What You'll Do Coordinate the RAI process so resident assessments are completed accurately and on timeMake sure MDS assessments reflect each resident's physical, mental, and psychosocial statusKeep the facility in compliance with Medicare, Medicaid, OBRA, and other payer requirementsVerify documentation supports the services provided and reimbursement accuracyWork with the interdisciplinary team on care planning, assessment completion, and case managementTrack assessment schedules to make sure regulatory timelines are metSupport quality improvement work and survey readiness Required What you'll need: Graduate of an accredited school of nursing, with a current RN license in IllinoisAt least 2 years of MDS 3.0 and case management experience in a skilled nursing or post-acute settingComfortable working independently while coordinating with an interdisciplinary teamWorking knowledge of Microsoft Office Preferred Long-term care or skilled nursing experienceCurrent RAC-CT certification, or willingness to obtain it within 6 months of hire and keep it current Pay And Benefits $36.50–$49.00/hour, based on experienceMedical, dental, and vision insurance403(b) retirement savings planDaily PayPaid time offTuition reimbursement programWellBeing mental health support24/7 chaplain counseling365 program for new team member development Schedule Full-time, 8:00AM - 4:00PM Next Steps Apply today.
