MDS Coordinator RN

About the role

We are currently searching for a MDS Coordinator to join our friendly, caring and supportive team. Rehabilitation Center of Independence West Campus is rapidly growing and our team is looking to invest in a MDS Coordinator by providing opportunities to further your career and with the tools and encouragement you need to succeed.

Responsibilities

• Give your team the knowledge they need to care for each resident’s unique needs.
• Ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure our residents are comfortable and safe.

Requirements

• Minimum of an unencumbered State of Iowa R.N. License.
• Graduate of an accredited nursing program.
• C.P.R. Certification.
• Prior experience as a MDS Coordinator is preferable but not required.
• One (1) year of experience as an R.N. in a Skilled Nursing Facility setting.
• One (1) year of experience as a C.N.A.

Qualifications

• Knowledge of MDS (Medicare) and/or SNF (Skilled Nursing Facility) documentation.
• Experience in long-term care settings.
• Strong organizational and communication skills.
• Ability to work independently and as part of a team.
