Responsibilities
- Coordinates MDS assessments, CAAs, and care plans under CMS RAI regulations, working with the interdisciplinary team, with occasional staff-nurse duties.
- Works closely with the interdisciplinary team to ensure accurate and comprehensive assessments and care plans.
- Ensures compliance with Medicare and Medicaid regulations regarding MDS documentation and care planning.
- Collaborates with other departments to ensure seamless patient care coordination.
- Occasionally performs staff-nurse duties as needed to support the facility's operations.
Requirements
- Active RN license.
- Two years of nursing experience.
- Geriatric experience preferred.
- Strong organizational and communication skills.
- Ability to work independently and as part of a team.
- Familiarity with healthcare regulatory requirements.
- Proficient in using MDS assessment tools and software.
- Excellent interpersonal and problem-solving skills.
- Knowledge of long-term care and geriatric care principles.
