MDS Coordinator/LVN (Remote Position)

About the role

Under the direction and supervision of the Director of Nursing Services, the Medicare/MDS Coordinator coordinates the Medicare/MDS resident assessment process in accordance with State and Federal regulations, ensures timely completion of MDS assessments by the Interdisciplinary Team (IDT), and oversees the development, implementation, and evaluation of resident care plans.

Responsibilities

• Notify and coordinate the Interdisciplinary Team (IDT) for MDS assessment completion.
• Coordinate and ensure timely completion of MDS assessments by the IDT.
• Coordinate development, implementation, and evaluation of resident plans of care.
• Administer or implement treatments, medications, or other nursing interventions as indicated by the resident plan of care or physician orders.
• Provide nursing care in accordance with infection control standards.
• Follow safety policies while performing nursing care.
• Initiate emergency measures according to center policy and within standards of nursing practice.

Requirements

• Current LVN License in the state of California.
• 2 years of experience as an MDS Coordinator in a Skilled Nursing Facility.
• Background check with no history of abuse.
