MDS Coordinator/LVN

Responsibilities

• Notifies and coordinates the Interdisciplinary Team (IDT) for Medicare/MDS assessment completion.
• Ensures the IDT completes the MDS Assessment in a timely manner.
• Captures and implements the plan of care.
• Administers or implements treatments, medications, or other nursing interventions as necessary.
• Provides nursing care in accordance with infection control standards.
• Fulfills safety policies while performing nursing care.
• Initiates emergency measures as required by center policy and nursing practice standards.

Qualifications

• Bachelor’s degree in Nursing (BSN).
• Current state licensure as a Registered Nurse (RN).
• Experience in skilled nursing facility setting preferred.
• Knowledge of Medicare/MDS guidelines and regulations.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
