About the role
Brewster Village is seeking a MDS Coordinator to join our team. This role is responsible for overseeing and coordinating the federally mandated resident assessment instrument (RAI) process, including the MDS assessment, care area assessment, and care plan development.
Responsibilities
- Coordinate the RAI process, including MDS assessments, care area assessments, and care plan development.
- Oversee the completion and transmission of assessments to the national repository.
- Review final validation reports and correct or modify assessments as needed.
- Develop and revise comprehensive care plans according to regulatory requirements.
- Maintain compliance with state and federal guidelines.
- Collaborate with interdisciplinary teams to determine skilled levels of care and procure necessary documentation.
- Coordinate care with case managers for villagers using various insurance types.
- Maintain confidentiality of sensitive information.
- Assist in audit and improvement of staff education/competency.
Requirements
- Bachelor's degree in Nursing.
- Valid unrestricted RN license from the Wisconsin DSPS.
- RAC-CT certification preferred.
- Successful completion of a Post Offer Pre Employment Examination (POPE).
- Flu vaccination required.
