Responsibilities

- Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines
- Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans
- Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy
- Monitor assessment schedules and update MDS documentation as residents' conditions change
- Participate in care plan meetings and contribute clinical expertise to improve resident outcomes
- Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates
- Work closely with leadership to maintain compliance and promote continuous quality improvement
Qualifications
- Associate's or Bachelor's Degree in Nursing
- Current, unrestricted New Jersey Registered Nurse (RN) license
- Previous long-term care experience preferred
- Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred
- Strong clinical assessment, communication, and organizational skills
- Knowledge of federal and state MDS regulations
