Responsibilities: Coordinate and conduct MDS assessments, care plans, and quality measures for residents in our skilled nursing facilityEnsure timely and accurate completion of MDS assessments and care plans, meeting established deadlines and quality standardsCollaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement individualized care plansMaintain accurate and detailed records of resident assessments, care plans, and quality measuresAnalyze data and identify trends to improve care processes and outcomesProvide education and training to staff on MDS and care planning processesParticipate in quality improvement initiatives and quality management activitiesEnsure compliance with federal, state, and accrediting agency regulations and standards Requirements: Minimum Requirements: Minimum 2 yrs of experience.
