MDS Coordinator

About the role

As a skilled and dedicated MDS Coordinator, you will play a crucial role in upholding our organization's mission to care for people with mutual respect, team spirit, and hard work.

Responsibilities

• Cook up the RAI process, including scheduling, data collection, and completion of MDS assessments
• Ensure accurate and complete MDS assessments, while also addressing any additional documentation requirements
• Implement effective care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each resident
• Maintain accurate and up-to-date records, including resident assessments and care plans
• Collaborate with staff to identify and address resident care needs, and implement changes as necessary
• Participate in quality improvement initiatives to improve resident outcomes and care processes
