About the Role
Assess residents’ physical and mental function, document on minimum data set forms completely and accurately, and document all additional required assessments. Determine appropriate referrals to other healthcare professionals and use resident assessment protocols to decide whether to proceed.
Responsibilities
- Interview residents, residents’ representatives, and others to gather information for minimum data set assessments.
- Examine residents to collect data for minimum data set assessments.
- Enter data into the computer program for minimum data set assessments.
- Print minimum data set assessments and related reports from computer programs and state reporting sites.
- Review reports on timely completion and scoring of minimum data set assessments and care plans, taking follow-up action as needed.
- Participate in the Quality Assurance program for the nursing department and cooperate with the Quality Assurance Coordinator.
- Participate in and/or conduct interdisciplinary team meetings and rehabilitation utilization meetings.
- Complete minimum data set assessment schedules and communicate them to the interdisciplinary team.
- Complete assignments accurately and adhere to CMS-established time schedules.
- Reconcile end-of-month and end-of-quarter reports.
- Establish open lines of communication between all nursing personnel.
- Plan, organize, develop, implement, and interpret the plan of care process, coordinating care conference meetings.
- Demonstrate understanding of case mix and maximization for reimbursement.
- Understand and apply medical diagnostic coding.
- Maintain knowledge of nursing and medical practices, procedures, and relevant laws, regulations, and guidelines for nursing care facilities.
- Work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families, therapy staff, and consultants.
- Relay information concerning a resident’s condition.
Requirements
- Current, unencumbered, active license to practice as a Licensed Practical Nurse or Registered Nurse in this state.
- Supportive team member who contributes to and exemplifies teamwork.
- Ability to work harmoniously with other personnel.
- Must not pose a direct threat to the health or safety of others in the workplace.
