MDS Coordinator
Responsibilities
- Coordinate the development, implementation, and maintenance of Individualized Comprehensive Assessment Plans (ICAPs) and other assessment plans as required
- Conduct and document standardized assessments, including Minimum Data Set (MDS) assessments, on all residents
- Ensure accurate and timely completion of MDS assessments and updates
- Collaborate with healthcare team members to identify and address resident care issues
Requirements
- RN/LPN
- Experience in a long-term or short-term care setting
- Strong communication, organizational, and leadership skills
- Ability to work effectively in a fast-paced environment with minimal supervision
- Proficiency in Electronic Medical Records (EMRs) and MDS software
