MDS Coordinator
Responsibilities
- Coordinate the development and maintenance of resident Assessment Care Plans (ACPs) and Minimum Data Set (MDS) assessments
- Accurately and timely complete MDS assessments, care plans, and changes to MDS assessments
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Conduct regular assessments and revise care plans as needed
- Maintain accurate and up-to-date records of resident care and services
- Participate in quality improvement initiatives and quality assurance activities
- Provide education and training to staff on MDS assessment and care planning
- Monitor and report on resident outcomes and quality indicators
Requirements
- Minimum of 2 years of experience as a MDS Coordinator
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Proficiency in electronic medical records and MDS software
- Ability to maintain confidentiality and adhere to HIPAA guidelines
- Florida Nurse license
