MDS Coordinator

About the role

We are currently searching for a MDS Coordinator to join our friendly, caring and supportive team at Concord Care Center. Concord Care Center is rapidly growing and our team is looking to invest in a MDS Coordinator by providing opportunities to further your career and with the tools and encouragement you need to succeed.

Responsibilities

• Give your team the knowledge they need to care for each resident’s unique needs.
• Ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure our residents are comfortable and safe.

Requirements

• Minimum of an unencumbered State of Iowa R.N. License or LPN certificate.
• C.P.R. Certification.
• Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting, and (1) year of experience as a C.N.A. is preferable but not required.

Qualifications

• Graduate of an accredited nursing program.

Skills

• Knowledge of nursing practices and procedures.
• Attention to detail.
• Ability to communicate effectively with residents, families, and healthcare providers.
