About the Role
A well-established long-term care organization is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team. The ideal candidate is detail-oriented, organized, and knowledgeable in MDS processes and regulatory requirements. This position plays a key role in ensuring accurate clinical documentation, reimbursement, and quality resident care.
Responsibilities
- Complete accurate MDS assessments and comprehensive care plans in accordance with regulatory requirements.
- Initiate and update resident care plans while collaborating with the interdisciplinary care team.
- Prepare and distribute monthly care plan schedules in a timely manner.
- Maintain and update resident assessments and care plans as required.
- Monitor and audit clinical documentation to ensure accuracy, completeness, and compliance.
- Identify documentation concerns and communicate ongoing compliance issues to nursing leadership.
- Maintain strict confidentiality of resident, employee, and organizational information.
- Support quality assurance initiatives and contribute to positive survey outcomes.
Requirements
- Prior hands-on experience with MDS 3.0 is required.
- Current Registered Nurse (RN) license in the applicable state.
- Previous long-term care or skilled nursing experience is required.
- Strong organizational, time management, and critical thinking skills.
- Proficient with electronic medical records and computer applications.
- Excellent verbal and written communication skills.
- Ability to work collaboratively as part of an interdisciplinary team while managing multiple priorities.
