About the role
We are seeking an experienced MDS Coordinator to join our team at Autumn Lake Healthcare. Our company is dedicated to providing a supportive and nurturing environment for our staff and residents.
Responsibilities
- Develop and implement care plans based on the MDS assessment
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Ensure accurate documentation and compliance with regulatory standards
- Communicate regularly with residents and families regarding care progress
- Participate in staff meetings and training sessions
Qualifications & Experience Requirements
- Must have, as a minimum, 1 year(s) of experience as a MDS Coordinator
- Must possess, as a minimum, a Nursing Degree from an accredited college or university
- Must have, as a minimum, 1 year(s) of experience in a hospital, long-term care facility, or other related health care facility
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Maryland
- Must hold a valid LPN license if applicable
