Supportive work environmentOpportunity to make a real impact on residents' livesHealth, dental, and vision benefits (for eligible positions)Career growth opportunities Responsibilities: Gather information on our skilled nursing facility's current and future patients for future assessment, including physical and mental states.Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placementDetermine potential Resource Utilization Groups (RUGs) and expenses associated with a potential admissionReview all applications for admission, followed by on-site or phone assessment, and communicate results of assessment to the Admissions CoordinatorComplete and assure the accuracy of the MDS process for all residentsMaintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicating changes in regulationsMonitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursementFacilitate problem-solving for complicated admissionsMonitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submissionAssess charts and communicate with healthcare teams to create applicable health care plans for their current and incoming residents.
