MDS Assessor

About the role

Park Nursing Home is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Rockaway Park, NY. The 196‑bed facility is located right by the boardwalk at Rockaway Beach, offering a beautiful view of the Atlantic Ocean and convenient access to the subway, water ferry, and buses.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non‑compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements

• Registered Nurse (RN) License (New York)
• MDS Experience Preferred
• Long Term Care Experience Required
• Highly organized, professional & motivated
• Solid computer skills
• Excellent communication skills
• Friendly and a team worker
