MCC HIV Case Manager

About the role

The HIV Case Worker (CW) addresses patients’ socioeconomic needs and assists the Medical Care Coordination (MCC) team with patient monitoring, service linkages, care-plan updates, follow-ups, and outcome tracking. The CW also serves as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure seamless, timely enrollment of new patients into medical care.

Responsibilities

• Conduct outreach and presentations to underserved patient populations on HIV, Sexually Transmitted Diseases/Infections (STD/STI), and related topics (parenting, reproductive health, contraception, men’s health, family communication about sexuality, and accessing SJCH services).
• Address patients’ socioeconomic needs and assist the MCC team with patient monitoring and follow-up on service linkages.
• Update care-plan results, follow up with patients, and track outcomes.
• Act as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure seamless, timely enrollment of new patients into medical care.
• Link patients to care and support services within and external to St. John’s.
• Organize and maintain complete, accurate records and file reports in a timely manner.
• Adhere to HIPAA statutes, client confidentiality, and agency policies.
• Assist with daily program operations and general office duties.

Requirements

• Bachelor’s degree in a related field (human services, health services, or similar).
• Comfortable working with diverse populations, including formerly incarcerated individuals, undocumented individuals, LGBT populations, communities of color, and those experiencing homelessness, substance abuse, and/or mental illness.
• Two years’ related experience in a health or social service setting (preferred).
• At least two years’ experience conducting community and/or street outreach.
• Experience with community organizing.
• Excellent oral/written communication, interpersonal, and presentation skills.
• Bilingual—written and spoken Spanish/English (required).
• Strong computer skills.
• Family Planning Health Worker Certification or similar (may be obtained after hire).
• Rapid HIV Counselor certification (may be obtained after hire).
