Case Manager HIV
About the role
Southwest Care Center (SCC) is a non-profit healthcare organization with over 25 years of experience providing exceptional healthcare services in Santa Fe and Albuquerque, NM. As an FQHC-LAL, SCC seeks a Case Manager to join their team of professionals serving patients in their clinics.
Responsibilities
- Must have a strong desire to make a difference in the community and embrace a team-oriented approach.
- Works with health care team members to promote interdisciplinary treatment planning.
- Conducts intake interviews, performs psychosocial assessments, identifies risk factors, and performs eligibility and referral services.
- Maintains eligibility in health insurance plans and patient assistance programs, and follows up with patients and agencies.
- Links patients to a broad range of services, including medical care, behavioral health, substance abuse treatment, financial assistance, housing, food, financial entitlement, clothing, transportation, vocational training, and harm reduction services.
- Maintains appropriate knowledge of grants and regulatory requirements, and ensures documentation is consistent with meeting such requirements.
- Develops and maintains relationships with community resources to stay updated on available resources.
- Assists with Outreach HIV/HCV rapid testing and counseling programs (certification required).

Pay
The salary range for this position is $40,000 - $60,000 annually, commensurate with experience.
Schedule
The Case Manager position is full-time, with a regular weekly schedule of 40 hours.
