About the role
The Maternity Group Home Case Manager is responsible for providing comprehensive support to expectant mothers and their families in a group home setting. This includes ensuring the health and safety of the residents, coordinating with healthcare providers, and assisting with family planning and education.
Responsibilities
- Provide individualized case management services to expectant mothers and their families
- Coordinate with healthcare providers to ensure access to prenatal care and other necessary services
- Assist with family planning and education
- Ensure the physical and emotional well-being of residents
- Collaborate with other agency staff and community resources
Requirements
- Bachelor’s degree in social work, public health, or related field
- At least 2 years of relevant experience in maternal and child health or related field
- Valid driver’s license and reliable transportation
- Ability to work flexible hours including evenings and weekends
Qualifications
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
Skills
- Knowledge of maternal and child health issues
- Experience with case management and family support
- Excellent organizational and time management skills

Schedule
Monday through Friday, 8:00 AM - 4:30 PM
