Maternity Group Home Case Manager

About the role

The Maternity Group Home Case Manager is responsible for providing comprehensive support to expectant mothers and their families in a group home setting. This includes ensuring the health and safety of the residents, coordinating with healthcare providers, and assisting with family planning and education.

Responsibilities

• Provide individualized case management services to expectant mothers and their families
• Coordinate with healthcare providers to ensure access to prenatal care and other necessary services
• Assist with family planning and education
• Ensure the physical and emotional well-being of residents
• Collaborate with other agency staff and community resources

Requirements

• Bachelor’s degree in social work, public health, or related field
• At least 2 years of relevant experience in maternal and child health or related field
• Valid driver’s license and reliable transportation
• Ability to work flexible hours including evenings and weekends

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Skills

• Knowledge of maternal and child health issues
• Experience with case management and family support
• Excellent organizational and time management skills

Schedule

Monday through Friday, 8:00 AM - 4:30 PM
