Maternal Wellness Coordinator

This position is accountable for successfully implementing the program model, goals, and desired outcomes. The position provides clinical expertise and requires performing activities that benefit clients being served by the agency. The Maternal Wellness Coordinator provides support to the Program Director, facilitates support groups under the supervision of the Program Director, and provides screening of perinatal parents. The Maternal Wellness Coordinator works flexible hours to accommodate clients’ needs. The position requires being available for home visits, as needed. Local travel required. Essential Functions and Responsibilities: Facilitate peer support groups for perinatal parents (under the supervision of the Program Director) and deliver evidence-based interventions for which the employee is trained, including recruitment of participants, scheduling support groups, identifying locations for support groups and reserving spaces, arranging for food and refreshments, tracking pre- and post-tests for member of support groups, and collecting outcomes from participants. Provide evidence-based mental health screening tools to perinatal parents, interpret the results, and ensure parents are offered support to meet their needs.Provide case management services to perinatal families, including linking families to services, confirming appointments for registered participants, and establishing/maintaining relationships with other perinatal providers.Facilitate monthly support groups for women who have experienced perinatal loss.Develop and implement maternal mental health trainings and presentations for health professionals and community at large to effectively educate targeted audiences about maternal mental health and services available.Performing Outreach activities to promote HMHB’s support groups and mental health trainings.Develop and maintain relationships with healthcare providers to promote Circle of Moms including pediatric practices and other health organizations whose client/patient base may benefit from the program. Assist the Clinical Director and Program Director with managing Continuing Education Broker activities.Utilize HMHB’s internal data system for tracking client information and maternal mental health training participants Utilize HMHB’s internal data system to reserve conference room for in-person support groups and ensure space availability for scheduled sessions. Build partnerships and promotes flow of communication among partners to ultimately provide clients with appropriate services. Assess and analyze services and access barriers in Palm Beach County and bring to the attention of Senior Leadership. Effectively monitors program performance to ensure outcomes are being met. Develop reporting and ensure that program meets grant/funder requirements, and ensure all related reports are complete and timely. Attends pertinent meetings and trainings related to the position and as deemed by Supervisor. Knowledge, Skills & Abilities: Excellent interpersonal relational, problem solving and communication skills; both verbal and written.Knowledge of maternal and child health issues. Data collection and analysis skills. Computer skills including Microsoft Office Suite. Teaching, platform and presentation skills with adult learners necessary. Considerable knowledge of principles and practices of working with diverse populations. Ability to meet and communicate with diverse groups. Training in Mothers and Babies Curriculum preferred. Bi-lingual required. As needed, the Maternal Wellness Coordinator will translate, interpret, and/or review both written and verbal language to meet the needs of the clients and/or agency. Experience Required: Knowledge of clinical assessment and therapeutic interventions with pregnant and postpartum women. Experience navigating and linking clients to community services. Ability to work independently and as part of a team. Demonstrated flexibility and ability to work in various roles, settings and capacities while respecting boundaries in each role. Educational Requirements:Bachelor’s degree is required from an accredited university in social work, psychology, mental health or related social services field Preferred Credentials/Licenses: State of Florida Clinical License (LMHC, LCSW, LMFT) or License-eligible Physical Demands and Work Environment:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
