About the role

The Maternal Fetal Nurse Navigator supports, coordinates, and guides high-risk obstetric patients and their families within the health system. They collaborate with various healthcare team members to enhance patient care and outcomes for Maternal Fetal Medicine patients.

Responsibilities

  • Provides support and care coordination for high-risk obstetric patients and their families.
  • Collaborates with physicians, nursing, and imaging staff, and other healthcare team members to optimize patient care and outcomes.
  • Communicates results of diagnostic testing and procedures to patients and their families.
  • Coordinates follow-up appointments for patients requiring additional evaluation, testing, consultations, or procedures.
  • Performs insurance verification and patient account registration.
  • Manages all incoming and outgoing referrals for Maternal Fetal Medicine patients, ensuring services are scheduled and results are obtained.
  • Verifies insurance, registers patients, and obtains authorizations for clinic visits, imaging studies, procedures, and other services.

Requirements

  • Current, unencumbered license as a Registered Nurse in the state of Georgia.
  • Bachelor of Science in Nursing preferred.
  • BLS certification required.
  • Four years nursing experience preferred.
  • Experience with patient registration/admitting, diagnosis coding, and IBM PC experience preferred.

  • Comprehensive assessment of patient's physical and psychosocial status.
  • Effective and efficient coordination, implementation, and monitoring of the care management process and treatment plan.
  • Open and empathetic communication with patients and families.
  • Collaborative work with multidisciplinary care teams to develop and implement care plans.
  • Effective communication with diverse audiences including professionals, patients, community members, insurance companies, etc.
  • Knowledge of patient teaching skills.
  • Organization, self-motivation, initiative, and fiscal responsibility.

Pay

Salary range based on experience and qualifications.

Schedule

Full Time, 8 HR Day Shift, 8-5 M-Fri
