Responsibilities

  • Manages a case load of antenatal, postpartum, newborn, and pediatric home care patients.
  • Completes the nursing process: assessment, diagnosis, planning, implementation, and evaluation for patients using a family-centered and developmentally appropriate approach.
  • Collaborates with members of the interdisciplinary team, health care providers, and community services to ensure patient needs are met and utilization of services is effective.
  • Provides clinical oversight of LPNs and LNAs in the home, as necessary.
  • Develops a plan of care under the direction of the patient’s physician and in collaboration with the patient, their family and/or the primary caregivers upon admission.
  • Periodically reviews the patient care plan and recertifies it as appropriate.
  • Initiates referrals for interdisciplinary services and communicates with other disciplines in a timely manner.
  • Provides ongoing assessment of the patient’s clinical, functional, and mental status to identify problems and/or progress toward desired goals.
  • Communicates significant changes to the patient’s physician and clinical manager as appropriate.
  • Follows agency policy when mandatory reporting is required.
  • Completes nursing interventions and education relevant to the client.
  • Reinforces caregiver and patient education provided by other health care providers.
  • Accepts responsibility for assignment to perform specialized procedures and/or see patients in other programs as needed, including Home Health and Hospice, precepting of students and new staff, etc.

Requirements

  • Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school of nursing.
  • One year of experience as an RN in maternal or pediatric home health care setting is preferred.
  • Unencumbered RN license in VT and NH or compact.
  • Valid driver’s license.
  • Current car insurance that meets minimum standards.
  • BLS within 90 days of hire.
