Job Summary
As a Social Worker at Baylor St Luke's Medical Center, you will provide vital support and guidance to individuals and families facing complex challenges. Your responsibilities include conducting psychosocial assessments, developing care plans, providing counseling and crisis intervention, and connecting clients to resources. You will also advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
Qualifications
- Masters in Social Work and 1-year post-MSW experience or Social Work internship in a clinical or medical setting, upon hire
- Master Social Worker: TX, upon hire
- Minimum 3-year post-MSW healthcare experience (preferred)
Responsibilities
- Provide developmentally appropriate care for all populations served
- Advocate and educate on patient/family self-care management, health management education, bioethics referrals, and more
- Manage psychosocial issues such as crisis intervention, psychosocial assessment, counseling support, and referrals
- Facilitate patient/family care conferences and continuity/transition management
- Coordinate community resources and manage social determinants of health
Preferred Qualifications
- Minimum 3-year post-MSW healthcare experience
