Responsibilities

• Assessment & Care Planning: Interview participants to evaluate social, emotional, economic, and environmental needs, prioritizing issues to develop and document comprehensive intervention and post-discharge plans.
• Agency Collaboration & Referrals: Consult with internal management, statutory/voluntary agencies, and caregivers to ensure treatment compliance while connecting participants to community health, welfare, and social resources.
• Complex Case Management: Support concurrent reviews, discharge planning, home care, and ongoing social service coordination by building strong networks and identifying creative solutions for participant needs.

Requirements

• Master's degree in Social Work required.
• Preferred Credential(s): Driver’s License.
• 3 years of experience preferred.

$49,374.00 - $72,209.48 per year
