Mass Ability Case Coordinator

Full-Time | 40 Hours/Week | Monday-Friday, 9:00 AM-5:00 PM Salary: $65,000-$70,000 annually (paid biweekly) Northeast Arc is seeking a compassionate and organized MassAbility Case Coordinator to support individuals with disabilities in living independently within their communities. This role provides direct, in-home case management and coordination services, helping participants access housing, healthcare, benefits, transportation, community resources, and other supports that promote self-determination and independence. Key Responsibilities Provide individualized support and case coordination for assigned participants.Assist with housing, budgeting, public benefits, healthcare appointments, transportation, and community engagement.Help participants achieve goals outlined in their service plans.Connect individuals with community resources and support networks.Maintain accurate documentation and program records.Collaborate with participants, families, providers, and funding agencies.Participate in an on-call rotation as assigned. Qualifications Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or a related field preferred.Experience supporting individuals with disabilities or in human services preferred.Strong organizational, communication, and problem-solving skills.Ability to work independently and manage a caseload.Valid driver's license, reliable transportation, and ability to travel throughout the service area.
