Manager, SDoH
Description Summary/Position Objectives
This position is responsible for executing Social Determinants of Health (SDoH) vision for Maryland Physicians Care (MPC). The manager will lead the implementation of local initiatives aimed at addressing social conditions that impact health outcomes, develop a process to collect, analyze, and report data metrics, and ensure these cost-effective services are provided to members who need them the most.

Maryland Care Management, Inc. (MCMI) manages Maryland Physician Care's (MPC) statewide provider network of hospitals and physicians. MCMI is proud of its footprint in the community and has been providing services to the HealthChoice Medicaid populations since 1996, making it one of MD's largest Medicaid-managed care organizations with over 230,000 members.
Requirements
- Understanding of social determinants of health, health disparities, inequities, and social risk factors.
- Build strong relationships with key internal and external stakeholders through active participation in community-based initiatives.
- Maintain confidentiality and comply with the Health Insurance Portability and Accountability Act (HIPAA).
- Bachelor’s degree in public health, social work, behavioral sciences, or an equivalent education and demonstrated experience in social determinants of health. Master’s degree preferred.
- Three-plus (3+) years’ experience with population health, care coordination, and/or community-based programs.
- One to two (1-2) years’ management experience.
- Experience within a Medicaid/Medicare managed care environment.
- Valid MD Driver’s License.
Qualifications
- Knowledge and skills in managing and evaluating team member performance; providing coaching, consultation, employee development, and recognition.
- Ensuring ongoing, appropriate staff training; holding regular team meetings to drive good communication and collaboration.
- Formulating and updating departmental plans and priorities to address any business or operational challenges.
- Supporting and collaborating with the Sr. Director of Population Health to provide insight into specialty roles and implement changes as needed.
- Proactively identifying opportunities for improvement with assigned initiatives.
- Hiring, training, and managing field staff in assigned markets.
- Establishing and building key relationships with community-based organizations to execute SDoH strategy.
- Identifying opportunities to develop and pilot new SDoH initiatives.
- Gathering input and maintaining initiative-taking communication across business units to coordinate SDoH function across the market.

Pay
$88,000 - $97,000 annually.
