Manager, RN Care Management - Winter Garden

About the role

Oversees the daily operations of the Care Management department in inpatient, observation, and emergency department settings.

Responsibilities

• Ensures documentation and compliance for discharge planning, patient choice, and other regulatory requirements.
• Ensures staff participation in daily multidisciplinary rounds and actively participates to optimize the interdisciplinary care coordination.
• Makes sure staff documentation, reports, and dashboards adhere to the company-wide standard processes and quality care.
• Oversees care managers in providing comprehensive discharge planning from admission to discharge, ensuring timely and effective care management interventions to prevent delays and readmissions.
• Collaborates with hospital leadership and executives to address pace of care delays and ensure continuous progress in care and throughput.
• Identifies process improvement opportunities and actively participates in interdisciplinary meetings to enhance care coordination.
• Other duties as assigned.

Requirements

• Bachelor's degree [Required]
• Master's degree [Preferred]
• 1+ years of leadership experience [Required]
• 1+ years of care management experience [Required]
• 1+ years of care management leadership experience [Preferred]

Qualifications

• Formal de-escalation training within 90 days
• Licenses and certifications: Accredited Case Manager (ACM) [Preferred], Certified Case Manager (CCM) [Preferred]
