As Manager for Case Management, you will report to the Case Management Director and will be responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management within your assigned area of responsibility in alignment with the goals of the Case Management Department.

• Provide day-to-day oversight and management of the case management program, ensuring effective coordination, supervision, and administrative support of the case management team
• Act as a case management subject-matter expert, serving as a resource, advisor and leader for the members of the case management team
• Supervise and monitor professional and support staff, and ensure effective care coordination and case management practices consistent with hospital policies, applicable regulations and guidelines
• Evaluate the quality of interactions, documentation and assessments conducted by the case management staff and facilitate initiatives that promote quality outcomes and improve patient throughput while supporting a balance of optimal care and appropriate resource utilization

Qualifications

• Bachelor's in Nursing OR Master's in Social Work required
• 5 years of case management in an acute hospital setting required
• Thorough knowledge of Case Management defined by established rules and regulations of federal and non-federal programs, and understanding of hospital reimbursement as it relates to different payers such as Medicare, Medi-cal, Managed Care and private insurance plans
