You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. Responsibilities

• Lead and develop a team of Care Managers
• Supervise direct and indirect reports
• Drive process improvement initiatives
• Monitor quality outcomes and care gap closures
• Prepare and present operational updates to leadership
• Collaborate with providers and facility staff to improve outcomes

Requirements

• Bachelor’s Degree required
• Registered Nurse License (RN), LCSW, LCMHC, or LPC required
• 3 years of case management experience required
• 1 year of leadership experience required
• Knowledge of LTSS/HCBS or Medicaid requirements preferred

Pay

$101,000 – $168,000 annually
