Manager - Intake - Full-Time

Role: Manager - Home Health and Hospice Intake Responsible for leading and coordinating Hospice and Home Health Intake operations to ensure a high-quality, efficient experience for patients, families, referral partners, and caregivers. Initially overseeing Hospice Intake for the East region, the role includes driving process standardization, supporting caregiver engagement, optimizing resources, and monitoring key performance metrics. The manager collaborates with healthcare leaders, physicians, and referral sources to improve communication, streamline workflows, and support service growth across Hospice and Home Health sectors. Qualifications Graduate from a school of nursing; Bachelor's preferredActive RN license in the practice stateMinimum three years of nursing experienceLeadership or management experience preferredBLS certification required within 90 days of hire; CHPN certification preferredStrong human relations and communication skills; clinical expertise and understanding of patient care processes essential
