Manager - Homeless Outreach

About the Role

The Supervisor provides supervision of case managers, outreach workers, and peers with lived experience on the homeless outreach team. This role also performs admission and diagnostic assessments for people experiencing homelessness who want to access mental health and substance use services. The Homeless Outreach Team serves adults with severe and persistent mental illness (SPMI) through outreach, engagement, and intensive case management, with a focus on relieving street homelessness.

Responsibilities

• Provides supervision of the Homeless Outreach Team, community-based homeless outreach workers, and a Coordinated Entry Navigator position.
• Supervises Peers with Lived Experience of homelessness.
• Oversees the provision of case management services delivered by the team.
• Assists in outreach to people experiencing homelessness and responds to community stakeholders and members requesting assistance for those in a housing crisis.
• Completes admission intakes and diagnostic assessments for individuals experiencing homelessness who seek mental health and substance use services.
• Ensures timely documentation of services conducted by the team.
• Upholds standards for service delivery and quality.
• Coordinates with internal providers and community agencies to ensure appropriate follow-through of services for consumers.
• Conducts weekly group and individual supervision as needed.
• Provides education to staff on assisting consumers in obtaining safe and affordable housing.
• Develops and maintains positive landlord relationships.
• Assists in connecting consumers to available housing resources in the community.
• Models strengths and recovery-based services in the field with case managers.
• Participates in departmental meetings and other meetings related to case management and housing/homelessness services.
• Provides oversight and management of the PATH Grant, including grant requirements and deadlines.
• Handles data reporting in HMIS and PATH PDX for SAMHSA PATH Grant Outcomes.
• Provides leadership in Continuum of Care committees and other committees working to end or prevent homelessness.
• Maintains positive, effective relationships with community stakeholders, government leaders, and community organizations affecting resources available in the community.
• Participates in CoC and other community coalitions.
• Demonstrates understanding of the Housing First model.
• Acts as a steward of the organization’s resources, ensuring proper, legal, and ethical use.
• Develops and expands positive working relationships with referral sources and external partners while seeking additional community services and benefits for clients.

Requirements

• Master’s degree with a social service emphasis required.
• Qualified Mental Health Professional designation required.
• Familiarity with treatment approaches and needs of adults with severe and persistent mental illness (SPMI).
• Effective oral and written communication skills.
• Ability to establish effective working relationships with staff and community agencies.
• Sensitivity to cultural and ethnic differences.
• Ability to safely perform essential job functions with or without reasonable accommodations.
