Manager - Community Health/Community Access
Qualifications
Bachelor's degree required; Master's preferred
5+ years of community health experience
2+ years of supervisory experience
Work Environment
Hybrid schedule with 1-2 days/week in office with other days in the community or remote
Significant collaboration with community partners and internal teams
Flexible work hours based on community and business needs
Responsibilities
- Develops and adheres to departmental staffing, revenue and/or expense budgets.
- Responds to changes in the business which may affect the ability to achieve the budget goals.
- Develops and oversees programs targeted for health literacy in the community and in collaboration with select external partners.
- May assist in management of the health of select patient populations through advanced health assessments, evaluations, diagnosis and interventions.
- Provides health education and information to patient populations within the community and/or BJC.
- Supports the hospitals in serving their community and/or employees with preventative and educational health services.
- Complements BJC and/or individual system hospital’s public health strategy by providing much needed services.
- Captivates and coordinates BJC health literacy and screening initiatives for employees.
- Tracks, analyzes, and reports on relevant data.
- Aids in researching and planning new programs.
- Develops an annual comprehensive health plan for external clients as needed.
- Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans.
- Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
- Sets forth opportunities to pursue participation in research, planning and/or the evaluation of program components.
- Tracks, analyzes and reports on relevant data and disseminates findings appropriately.
- Implements findings and uses data to improve care of patients and the systems of healthcare delivery.
