Manager Community Clinical Support Services
Responsibilities
Captures and coordinates patient care needs, collaborating with the health care team to achieve desired clinical outcomes and manage care across the continuum.
Ensures adherence to professional and clinical standards among all areas supervised to achieve desired outcomes.
Demonstrates effective communication skills with the care team and works to eliminate communication barriers.
Serves as a liaison between physicians, families, nursing team and ancillary departments to ensure delivery of high-quality care.
Maintains clinical competency by ensuring the psychosocial and developmental needs of the patient are met, within a family-centered environment to promote safe and appropriate transitions of care.
Coaches and develops direct reports to ensure the delivery of high-quality patient care and that they are abiding by all policies and procedures of TCH and their license.
Ensures staff work within their scope of practice and clinical license as appropriate.
Qualifications
Bachelor's Degree in a clinical discipline aligned with their required license/certification
Master's Degree in Business, Healthcare Administration, Social Work or related field
3 years licensed/certified clinical experience
One of the following licenses:
- CCLS - Cert-Cert Child Life
- SCLST Association of Child Life Professionals (ACLP)
- MT-BC - Music Therapy-Board Cert
- Certification Board for Music Therapist
- ATR-BC - Art Therapist-Board Certified
- Art Therapy Credentials Board
- LMSW - Licensed Master Social Worker Texas Behavioral Health
- Texas Behavioral Health LMSW-AP - Licensed Master Social Worker - Adv Pract Texas Behavioral Health Ex Council (TBHEC)
- RN - Lic-Registered Nurses Texas Board of Nursing or Nurse Licensure Compact
