Manager - Case Management

Seeking a leader to manage daily operations of the Care Coordination department, including RN Case Managers, Social Workers, and administrative staff. Responsibilities Develop and implement departmental goals aligned with organizational strategy.Assess patient care quality and coordinate services among staff, physicians, and departments.Lead staff orientation, ongoing education, coaching, and ensure policy compliance.Identify and resolve process gaps, participate in quality improvement initiatives.Oversee departmental budgets, regulatory compliance, vendor relations, and contracts.Manage staff performance, satisfaction, conflict resolution, scheduling, recruitment, and payroll.Apply evidence-based practices to ensure patient-centered nursing care.Maintain a safe work environment and perform other duties as assigned. Qualifications Bachelor’s degree in Nursing or Master’s in Social Work.Minimum of five years’ experience with two years in leadership roles.Strong skills in team management, communication, process improvement, and budget oversight.Physical ability to perform job functions, including lifting, standing, and data entry.Professional licensure as an RN, LCSW, or LMSW valid in relevant states. Preferred Skills: MSN or MBA, case management, leadership experience.
