Manager - Case Management

Role Overview: Responsible for daily oversight and coordination of case management and discharge planning to ensure quality patient care, regulatory compliance, and efficient patient flow. Key Responsibilities: Collaborate with multidisciplinary teams, interpret payer guidelines, and utilize clinical decision-making skills. Ensure discharge plans meet state and federal regulations, using tools like Interqual or Milliman guidelines. Monitor and improve processes, performance, and staff development. Qualifications & Skills: Minimum of three years in case management and two years in a leadership role. Nursing degree (RN or BSN) required; case management certification preferred. Must demonstrate critical thinking, effective communication, decisive judgment, and the ability to work independently in high-pressure environments. Knowledge of hospital information systems and performance improvement techniques is essential.
