Overseeing daily home-based primary and palliative care services.

Responsibilities

• Manage scheduling, care team logistics, and site workflows to ensure safe, efficient in-home and telehealth visits for complex patients.
• Track quality, productivity, and regulatory compliance to maintain high standards.
• Optimize operational processes, manage budgets, and maintain vendor relationships.
• Support electronic health records (EHR) systems and reporting functions.
• Partner with clinical leaders to foster a patient-first, team-oriented culture.
• Mentor staff and drive continuous improvement across assigned sites or markets.
