About the Role

A Licensed Vocational Nurse (LVN) administers hospice skilled nursing care to patients on an intermittent basis in their place of residence, following physician orders and the plan of care under the direction and supervision of a Registered Nurse. The LVN develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s), and participates in patient, caregiver, and family counseling and education.

The LVN communicates with all physicians involved in the plan of care and other healthcare practitioners as appropriate, provides physician-ordered services, and contributes to the agency’s Quality Assessment and Performance Improvement (QAPI) program. The LVN is monitored and evaluated by the Registered Nurse (RN) for service provision, treatments, patient education, and communication regarding patient status and needs.

Responsibilities

  • Administer hospice skilled nursing care in patients’ residences.
  • Develop and evaluate the plan of care in collaboration with the patient, caregiver(s), and RN.
  • Participate in patient, caregiver, and family counseling and education.
  • Communicate with physicians and healthcare practitioners regarding the plan of care.
  • Provide services as ordered by the physician and outlined in the plan of care.
  • Contribute to the agency’s QAPI program and participate in interdisciplinary patient assessments.
  • Perform treatments, patient education, and data collection as delegated by the RN.
  • Demonstrate adherence to agency policies in service delivery.

Requirements

  • Current LVN license in the state(s) of practice.
  • Minimum of one (1) year of nursing experience (preferred).
  • CPR certification (American Heart Association or American Red Cross).
  • Ability to exercise initiative and independent judgment.
  • Strong oral and written communication skills, with excellent interpersonal abilities.
  • Demonstrated customer relations skills and commitment to quality service.
  • Valid driver’s license, automobile liability insurance, and reliable transportation.
  • Acceptance and support of the agency’s core values and goals.
  • On-call availability at least once per month (may be required).
  • 100% travel required.
