About the role
This PRN (as needed) position supports hospice patients across our service area as part of the Hand in Hand hospice program, a part of Arc Hospice & Palliative Care. Assignments may vary based on patient needs and coverage requirements. The Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) provides direct nursing care to patients in their homes or care settings under the supervision of a Registered Nurse, supporting the delivery of hospice care in accordance with the plan of care, agency protocols, and the nurse's scope of practice.
Responsibilities
- Provide direct nursing care to hospice patients under the supervision of a Registered Nurse, in alignment with the plan of care and scope of practice.
- Perform scheduled and unscheduled nursing visits, including comfort care, medication administration, and skilled observation.
- Document all care and communication accurately and promptly in the electronic medical record (EMR).
- Support pain and symptom management interventions, reporting changes in condition to the supervising RN or physician as appropriate.
- Collaborate with interdisciplinary team members to ensure coordinated, patient- and family-centered care.
- Educate patients and families on care techniques, medications, and symptom awareness as directed.
- Provide support for emergency visits, tuck-in calls, respite, and Continuous Care as appropriate.
- Participate in on-call rotations and respond to after-hours clinical needs when scheduled.
- Maintain compliance with agency policies, hospice regulations, and professional standards of practice.
- Perform other duties as required.
Qualifications
- Current LPN or LVN license in good standing in the state of practice or a valid compact license covering all states of operation.
- Minimum of one (1) year of clinical nursing experience preferred, ideally in hospice, home health, or geriatrics.
- Demonstrated understanding of hospice philosophy, symptom management, and end-of-life care practices.
- Proficiency in electronic medical record (EMR) documentation and mobile clinical tools.
- CPR certification required.
- Valid driver’s license, reliable transportation, and proof of automobile insurance.
- Strong communication skills and the ability to function effectively as part of an interdisciplinary care team.
- Must demonstrate a strong commitment to Arc Hospice & Palliative Care’s mission and values.
Pay
$28—$28 USD per hour
