Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Deliver skilled nursing services per prescribed orders
- Monitor physiologic and psychosocial changes
- Document diagnoses and interventions
- Provide compassionate, compliant nursing care in patients' homes
- Assess patient conditions, analyze health data, and collaborate with healthcare teams
Qualifications
- Current LVN license
- TB or Chest X-Ray
- BLS certification
- One year of professional nursing experience (preferred)
- Experience with enteral tubes, tracheostomy, or ventilator management is a plus but not mandatory—training provided if needed
Skills
- Strong assessment, critical thinking, and documentation skills
- Ability to work independently and adapt to patient needs
