Role Overview
Key Responsibilities
- Identify high-risk patients to reduce hospitalizations and ER visits.
- Perform health assessments, medication reconciliation, motivational interviewing, and patient education for chronic conditions.
- Support inpatient-to-outpatient transitions, including assessments, contingency planning, follow-up scheduling, and social referrals.
- Care coordination, documentation in electronic records, participation in team meetings, and report preparation.
- Perform clinical duties within the scope of an LVN/LPN license and stay current with evidence-based practices.
Qualifications & Skills
- Valid LVN/LPN license and BLS certification.
- At least 2 years of experience in clinical or hospital settings.
- Knowledge of chronic diseases such as COPD, asthma, diabetes, CHF, and IHD.
- Strong communication, organizational, and interpersonal skills.
- Proficiency with electronic medical records and computer skills.
- Ability to work independently in a less structured environment.
Preferred Skills
- IV certification, patient education, motivational interviewing, and outbound communication experience.
- Fluency in English and Spanish.
