Role Overview

The Healthcare Coordinator supports high-risk patients in navigating the healthcare system, acting as an educator, resource, and advocate to enhance quality of life.

Responsibilities

• Collaborate with care teams to identify high-risk patients
• Support chronic disease management through assessments, medication reconciliation, and patient education
• Assist with transitions from inpatient to outpatient care
• Ensure follow-up care and coordinate care plans
• Document accurately
• Participate in team meetings

Qualifications & Skills

• Licensed Practical/Vocational Nurse with active licensure
• BLS certification
• 2+ years of clinical experience
• Knowledge of chronic diseases
• Strong communication, organizational, and computer skills
• Preferred: IV certification, patient education, and bilingual proficiency in English and Spanish
