LVN - Full-Time and Part-Time

About the Role

This position involves providing quality home health nursing care to individual patients, ensuring safe and effective delivery of services. You will communicate regularly with RN case managers and physicians about patient needs and assist in performing specialized procedures and implementing care plans.

Responsibilities

• Deliver safe, high-quality nursing care in a home health setting.
• Coordinate with healthcare team regarding patient condition updates.
• Assist RNs and physicians with procedures and care plan execution.

Requirements

• Graduate of an accredited practical (vocational) nursing program.
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
